Not always. If your employer has 20 or more employees and you have active employer health coverage, you can usually delay Medicare Part B without a late-enrollment penalty. If your employer has fewer than 20 employees, Medicare typically becomes the primary payer at 65 and you'll want to enroll. What is the Medicare Part B late-enrollment penalty?
If you don't enroll in Part B when you're supposed to — and you don't have qualifying employer coverage — you may pay a 10% penalty for every 12 months you delayed. That penalty is added to your Part B premium permanently. It's a common, costly mistake, and it's avoidable with the right timing.
When is my Initial Enrollment Period (IEP)?
Your Medicare Initial Enrollment Period is a 7-month window around your 65th birthday: the 3 months before your birthday month, your birthday month, and the 3 months after. Enrolling early (in the 3 months before) usually means coverage starts the first day of your birthday month.
